Is Online Psychiatry Covered by Japanese Health Insurance?
Yes. Insured online psychiatric care runs through the same national health insurance as in-person care, at the same standard cost-sharing. Seeing your psychiatrist by video is not a premium service or an out-of-pocket workaround in Japan; it's ordinary insured medicine delivered through a screen.
A few parts of the picture deserve a closer look, because "covered" has edges.
How does coverage work?
The same way in-person coverage does: if your clinic accepts Japanese health insurance for in-person visits, its insured online consultations work the same way. The visit is billed to your insurance, you pay the standard percentage (30% for most working-age adults), and prescriptions issued online are filled at a pharmacy under the same coverage as always. Nothing about the video format changes your entitlement, and nothing about your nationality does either.
Insurers treat telemedicine as a delivery method, not a separate category of care, which is why there's no special enrollment, rider, or approval to arrange before your first online appointment.
What sits outside insurance?
Two kinds of extras can appear around an online visit, and it's fair to know about them in advance.
Communication and system charges. Clinics are permitted to charge a non-insured fee for the video platform and related administration. Whether and how a clinic does this is their published policy, so check the clinic's own page or ask when booking.
Delivery. If your medication is delivered rather than collected, a pharmacist still has to counsel you first over video, and that conversation is insured like the counter version it replaces. Neither changes the core: the medical care itself is insured.
Not every online service bills insurance at all. Some direct-to-consumer telehealth services are self-pay by design, so it is worth checking before you book whether the service takes your insurance card; what insured online care has to follow is set out in our telemedicine rules guide.
Does Jiritsu Shien Iryo apply to online visits?
Yes, at your registered clinic and pharmacy. The subsidy program that cuts your share from 30% to 10% attaches to your designated clinic and pharmacy rather than to the visit format, so insured online consultations at your registered clinic are handled the same way in-person visits are; confirm the arrangement with your clinic when you set up online care, since the program's paperwork runs through them. If you use delivery, the pharmacy that dispenses has to be one named on your certificate; medication dispensed anywhere else is billed at the ordinary rate.
