Is Online Psychiatry Covered by Japanese Health Insurance?
Online psychiatric care can be covered by Japanese health insurance, through the same insurance you use for an in-person visit, when the clinic bills insurance for it. The video format does not take the visit outside insurance. What can differ is the amount: the fee structure for an online visit differs from an in-person one in places, and a few extras sit outside insurance, as set out below.
How does coverage work?
Through the clinic's ordinary insurance billing: if your clinic bills health insurance for in-person visits, its insured online consultations go to your insurer too. You pay the insured share (30% for most working-age adults), and prescriptions issued online are filled at a pharmacy under your insurance as usual. Your entitlement depends on being enrolled in Japanese health insurance, not on your nationality.
There is no special enrollment, rider, or approval to arrange before your first online appointment.
What sits outside insurance?
Two kinds of extras can appear around an online visit:
- Communication and system charges: clinics are permitted to charge a non-insured fee for running the video system itself. Whether and how a clinic does this is its own published policy, so check the clinic's page or ask when booking.
- Delivery: if your medication is delivered rather than collected, a pharmacist still has to counsel you first by video, with the camera on; a voice-only call is not enough. That counseling is insured, like the counter version it replaces. What sits outside insurance is the shipping, plus any system fee the pharmacy charges for the call. Neither changes the core: the medical care itself is insured.
Not every online service bills insurance at all. Some direct-to-consumer telehealth services are self-pay, so check before you book whether the service bills your health insurance. What insured online care has to follow is set out in our telemedicine rules guide.
Does Jiritsu Shien Iryo apply to online visits?
Yes, at the clinic and pharmacy named on your certificate. The subsidy program that cuts your share from 30% to 10% attaches to your designated clinic and pharmacy, not to the visit format, so insured online consultations at your registered clinic fall under the program in the same way as in-person visits; confirm this with your clinic when you set up online care, since the paperwork runs through them. If you use delivery, the pharmacy that dispenses has to be one named on your certificate; medication dispensed anywhere else is billed at the ordinary rate.
