Antidepressants in Japan: What's Available, What's Not, and How to Switch

Most of the antidepressants prescribed in Western countries are available in Japan under the same generic names: sertraline, escitalopram, duloxetine, venlafaxine, mirtazapine, and more. The two big exceptions are fluoxetine (Prozac) and bupropion (Wellbutrin), which have never been brought to the Japanese market. If you take one of those, you'll need to switch after you arrive, and that is routine, manageable psychiatric work.

Which antidepressants are available in Japan?

  • SSRIs: sertraline, escitalopram, paroxetine, and fluvoxamine are widely prescribed. Fluoxetine and citalopram are not available.
  • SNRIs: duloxetine, venlafaxine, and milnacipran (approved for depression here but sold in the US only for fibromyalgia, as Savella).
  • Others: mirtazapine, vortioxetine, and trazodone (mostly used at low doses for sleep), plus older tricyclics like amitriptyline. The newest arrival is zuranolone, a rapid-acting antidepressant taken as a two-week course, launched in Japan in 2026.

Generics are standard in Japan, tightly regulated, and dispensed routinely; a box labeled with the generic name is normal practice, not a substitution to worry about.

What if your medication isn't available?

Switching usually follows the family tree:

  • Citalopram users move to escitalopram, the refined half of the same molecule.
  • Desvenlafaxine users move to venlafaxine, its parent drug.
  • Fluoxetine users switch to another SSRI; fluoxetine's long half-life means it tapers itself gently on the way out.
  • Bupropion has no direct relative here, so the plan depends on what it was doing for you and deserves an unhurried conversation.

Whatever you take, bring documentation: a letter or printout listing diagnosis, medications tried, doses, and response. And if you're still abroad, don't stop your medication in anticipation of the move; plan the bridge first.

Do Japanese doctors prescribe differently?

The drugs are the same, but approved maximum doses are sometimes lower than in the US (sertraline, for example, is approved up to 100 mg per day here versus 200 mg in the US), and titration tends to be cautious. If you arrive on a dose above Japan's approved maximum, your doctor will discuss options rather than simply copying the old prescription. None of this means weaker treatment; it means the first visit matters, so bring your history.

Can you bring your antidepressants into Japan?

For personal use, you may carry up to one month's supply of prescription medication with no paperwork; antidepressants are ordinary prescription drugs under Japanese import rules. For more than a month, apply in advance for an import certificate (輸入確認証, formerly yakkan shōmei); the health ministry publishes the rules in English.

A sensible arrival plan: land with up to a month of medication, book a psychiatrist for your first weeks, and hand over your records so the Japanese prescription starts before the suitcase supply runs out.
